MISCELLANIOUS INSURANCE FORMS
  • Broker of Record Letter
    Letter that assigns ARM-Capacity as broker of record on a policy allowing our insurance Specialists to get information from the insurance company and to provide services to the policy holder. The contents of this letter must be submitted on your company letter head.

  • Certificate of Insurance Request
    Document issued by the insurer as evidence that a policy has been issued with details on coverage. Additionally, this document request allows you to request an additional insured be added to the policy.

  • Loss Run Request
    Letter that is used to request updated loss run reports from your current and past insurance companies. The contents of this letter must be submitted on your company letter head.

  • Business Income Worksheet | PDF Version
    Excel Worksheet used to estimate your business income exposure that is often required by insurance companies to provide agreed value coverage.

  • Statement of Property Values | PDF Version
